How would I credit the following?

H.R. Pufnstuf
from
The World of Sid & Morty Krofft

That is how it's listed in the end credits.

FYI: This is a cast credit and no it's not for the H.R. Pufnstuf TV Show.

It's for "CHiPs" Green Thumb Burglar episode in which H.R. Pufnstuf appears as itself.

Since this is a character name what would be the correct parsing of the name? All in the first name field, such as for Madonna or The Rock or treat it as a real person's name. Which I would assume would be H./R./Pufnstuf.

Also how would I go about crediting the person that provides the voice of the character?
Since I can't see the person doing the voice I can't visually confirm who it is, so I would likely have to use some other source.

I just want some input, even though I think I have a sense of how to do it I want to make sure before I continue.

Given the way the krofts handled the character, i would list him according to standard Rules which would be H./R./Pufnstuf (H.R. Pufnstuf) to capture the actual On screen spacing, as Himself. IF we can find out who was inside the suit, thenhe could be credited as Joe Bolw....H.R. Pufnstuf (uncredited).

You are calling him a character, which to us (the viewers) he was, but according to the krofts he was the actor...playing himself. Niow yiou have me humming that theme...curse you.

If you bring up the Banana Splits I will get my voodoo doll out.

I agree, except that last bit: you're still confusing "credited as" with (uncredited) there. You've tried to argue that before, and Ken came in and set the record straight. Link to Ken's ruling here, but you'll need to plough through some more of the thread to see you "explaining" the same misconception. Suffice to say that you incorrectly wanted to use (uncredited) instead of "credited as" for Sandra Bullock there as well, but Ken repeatedly told us not to. So per Ken's ruling, if we know who's in the suit, it would be "Actor Name [H.R. Pufnstuf]" with NO need for an (uncredited) entry. But I don't think we actually know who's in the suit here, so then it doesn't even apply... I was just slightly worried that you still seem to be misunderstanding this.

I would enter it as 'H.R. Pufnstuf as Himself'.  I have no real opinion on the parsing...could be as a stage name or as a real name.  If you can find the actor in the suit, it would be 'Actor Name (H.R. Pufnstuf) as Himself'.

As for the voice actor, assuming it is a different person, this would be similar to the David Prowse/James Earl Jones situation.  Just list the voice actor, with the role 'H.R. Pufnstuf', and tick the 'voice' and 'uncredited' boxes.

You mean H.R. Pufnstuf is a character? He isn't real? Oh, the childhood dreams you have just shattered!
