When you open the Reports tool, in the bottom right corner there is an option to restrict the report to either boxsets contents or parent profiles only. I think you want the "Box Set Cases" option.

Media type is a bit more complicated but possible.
What you need to do is create a text box that contains the word "DVD" but add a Condition (at the bottom of the list of values on the left when you have the text box selected) that it is only visible when Media Type matches DVD.
Then directly over the top of that text box add another one that contains the caption "Blu-Ray" and this time make it visible only when media type matches Blu-ray.
