John Meredyth Lucas (May 1, 1919 - October 19, 2002) was a writer, primarily for television. He is best
remembered for the work he did on Star Trek as a writer, producer and director. He was the only writer
on the original series to direct an episode he penned ("Elaan of Troyius"). He also wrote for Mannix.

He was the son of screenwriter Bess Meredyth and writer/director Wilfred Lucas, and the adopted
son of director Michael Curtiz.

I see nothing in the information you quoted that referes to him as Mr. Meredyth Lucas. The fact that his mother's maiden name is part of his name has no bearing on how the name is parsed. Without further proof, I would parse him J/M/L

I agree... there is nothing there that shows it is a double last name... for all we know his parents gave him his mother's maiden name for a middle name. Definitely I have seen people do that before... on more then one occasion. So without further proof... I would do J/M/L as well.

Googling his name i find multiple sources that when they refer to him by his last name only use Lucas.

Example:

I daresay he used his middle name professionally to distinguish himself from the other John Lucas' in Hollywood.  There are at least a dozen if you believe iMD*.

I agree with Pete and Kevin.  There is no evidence that it is a double last name.

I agree with the others.  According to this article on StarTrek.com, which I would consider to be an authoritative source, they refer to him by his last name, Lucas
